Dear Parents/Guardians,
Welcome to the new school year! I hope everyone had a happy and healthy summer. I would like to take this opportunity to provide some tips that will help you and your student start the school year safely and smoothly.
•ALL students must have a green health information form on file in the health office. Please complete front and back of the form and sign the form on the back. You can pick this up at Open House or you will receive one sent home with your student after the first day of school.
•All over-the counter and prescription medications must be provided to and signed in with the school nurse by the parent or guardian before the medication can be administered. Medications must be replaced once they expire. Students may NOT turn in medications to the health office. Prescription medication must be provided in the original container with the pharmacy label attached. Controlled substances must be counted when they are dropped off.
•Emergency medications MUST be turned in by the first day of school with an action plan. These can be dropped off with the nurse at your student’s Open House on July 31st or August 1st.
•Students in Kindergarten and 1st grade should bring an extra set of clothes. Accidents happen and this helps reduce interruption in your student’s day.
